It is vital that the practice has an up to date record of your contact details, so we can contact you about your health care and treatment. Therefore, please contact us as soon as possible if your address, landline or mobile telephone number changes.
If you move to a new address outside of our practice catchment area, we will advise that you must register at an alternative practice. Unfortunately, there are no exceptions to this and you will be given 30 days to make alternative arrangements.

Our practice catchment area used to include Banks in West Lancashire. When this was changed, we agreed to retain those patients who lived in Banks. However, we regret that we are unable to register any new Banks families.
