Our practice catchment area covers Marshside, Crossens and Churchtown, and is bordered by Marine Drive (to the North), Park Road West (to the West), Roe Lane and Moss Lane (to the South) and Three Pools Waterway (to the East).

PLEASE NOTE THAT CURRENTLY, BECAUSE OF AN UNSUSTAINABLY LARGE INCREASE IN THE NUMBER OF PATIENTS REGISTERING AT THE SURGERY, WE ARE NOT REGISTERING NEW PATIENTS WHO ARE ALREADY REGISTERED WITH A GP SURGERY & STILL LIVE WITHIN THAT GP SURGERY'S CATCHMENT AREA.

 

If you would like to register to become a patient at our practice, please attend our reception in person - one of our receptionists will let you know what is required and give you the necessary forms to complete.

Alternatively, please download, print and complete the following three forms by hand, and bring them the surgery with photographic proof of your identity and your address.

Medical treatment is available from the date of initial registration - this is confirmed once one of the GP partners has signed the GMS1 form, which is usually the following working day.  Your previous practice should have given you 30 days to register at a new practice, which includes issuing any repeat prescriptions you require during that time.

You will be invited for a new patient check with one of our healthcare assistants.
